Liability Waiver and Release Agreement IMPORTANT: READ CAREFULLY BEFORE SIGNING. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS. This Liability Waiver and Release Agreement (the "Agreement") is made effective as of [Today’s Date], by and between Mother Sauce Productions, an LLC operating out of 1962 Market St., Denver CO 80204 (hereinafter referred to as "Entertainment Venue"), and the undersigned individual (hereinafter referred to as "signee"). 1. Attendance Agreement: By signing this Agreement, the signee acknowledges and agrees to abide by all rules, policies, and procedures established by the Entertainment Venue. 2. Assumption of Risks: The signee acknowledges that participating in activities or events at Mother Sauce Productions, whether indoors or outdoors, may involve certain risks, including but not limited to injury, property damage, or loss. These risks may arise from the actions or omissions of the signee or others, the condition of the premises, or the use of equipment, facilities, or services provided by Mother Sauce Productions. 3. Release of Liability: In consideration of being allowed to participate in activities at Mother Sauce Productions, the signee, on behalf of themselves, their heirs, executors, administrators, and assigns, hereby fully and forever releases, discharges, and holds harmless Mother Sauce Productions, its directors, officers, employees, volunteers, agents, and representatives, from any and all claims, demands, liabilities, actions, suits, or causes of action, whether at law or equity, for personal injury, property damage, or any other loss, including death, arising out of or in connection with the signee's participation at Mother Sauce Productions. 4. Maximum Liability of Mother Sauce Productions: If, notwithstanding the release of liability set forth in this Agreement, Mother Sauce Productions is found to be liable for any loss or damage, the signee agrees that the maximum liability of Mother Sauce Productions shall not exceed the membership fees paid by the signee for the current membership term. 5. Rights of Minors: If the signee is a parent or legal guardian of a minor (under 18 years of age), the signee agrees to assume all risks and responsibilities on behalf of the minor and to bind the minor to the terms of this Agreement. The signee further represents and warrants that they have the legal authority to enter into this Agreement on behalf of the minor. 6. Indemnity: The signee agrees to indemnify, defend, and hold harmless Mother Sauce Productions, its directors, officers, employees, volunteers, agents, and representatives from and against any and all claims, demands, liabilities, actions, suits, or causes of action, whether at law or equity, arising out of or in connection with the signee's actions, omissions, or participation at Mother Sauce Productions. This indemnity includes, but is not limited to, any claims for personal injury, property damage, or any other loss, including death, caused by the signee's negligence, recklessness, or willful misconduct. 7. Opting Out of the Waiver: If a signee wishes to opt-out of this waiver for a specific event or activity, they must submit a written request to Mother Sauce Productions at least 14 days prior to the event or activity in question. Mother Sauce Productions reserves the right to deny any opt-out requests at its sole discretion. 8. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of [State/Country], without regard to its conflict of laws principles.

Mother Sauce Terms and Conditions for Events By participating in events hosted by Mother Sauce Productions (the "Event"), you agree to be bound by the following Terms and Conditions. Please read these Terms and Conditions carefully before attending the Event. 1. Registration and Attendance: 1.1. Event registration is subject to availability and may require payment of a registration fee. 1.2. Attendance at the Event is subject to compliance with the registration process and any entry requirements specified for the Event. 2. Age Restrictions: 2.1. Some Events may have age restrictions. It is your responsibility to review the event details to determine if the Event is suitable for your age group. 3. Code of Conduct: 3.1. All attendees are expected to conduct themselves in a respectful and appropriate manner. Harassment, discrimination, or disruptive behavior towards others will not be tolerated. 3.2. Mother Sauce Productions reserves the right to remove any attendee from the Event who violates the code of conduct. 4. Media Release: 4.1. By attending the Event, you grant Mother Sauce Productions the right to use your name, likeness, voice, and any recordings or photographs taken during the Event for promotional and marketing purposes. 5. Event Changes or Cancellations: 5.1. Mother Sauce Productions reserves the right to make changes to the Event program, schedule, or venue. In the event of a cancellation or rescheduling, the Foundation will make reasonable efforts to notify attendees and provide alternatives, if possible. 6. Refunds and Cancellations: 6.1. Registration fees for Events are nonrefundable, except in the case of a complete Event cancellation by Mother Sauce Productions. 6.2. If you are unable to attend the Event, no refunds will be provided for registration fees. 7. Assumption of Risk: 7.1. Participation in certain Event activities may involve inherent risks. By attending the Event, you assume all risks associated with participation and agree to release Mother Sauce Productions from any liability for injuries or damages incurred. 8. Indemnity: 8.1. To the fullest extent permitted by law, you agree to indemnify and hold harmless Mother Sauce Productions, its directors, officers, employees, volunteers, agents, and representatives from and against any claims, demands, liabilities, actions, suits, or causes of action arising out of or in connection with your participation in the Event. 9. Liability Waiver: 9.1. Attendees may be required to sign a separate liability waiver before participating in certain activities at the Event. 10. Third-Party Content: 10.1. The Event may include presentations, materials, or content provided by third parties. Mother Sauce Productions does not endorse or guarantee the accuracy of third-party content. 11. Governing Law: 11.1. These Terms and Conditions shall be governed by and construed in accordance with the laws of [State/Country], without regard to its conflict of laws principles. 12. Amendments to Terms and Conditions: 12.1. Mother Sauce Productions reserves the right to amend these Terms and Conditions at any time. Any changes will be effective upon posting the revised terms on the Event website.

1. Event Cancellations or Rescheduling: 1.1. In the event of an unexpected cancellation or rescheduling of an event hosted by Mother Sauce Productions, the Entertainment Venue may, at its sole discretion, offer alternative arrangements or credits for future events. 1.2. If a patron is unable to attend a rescheduled event, no refunds will be provided, as the original ticket purchase is considered a donation. 2. Errors or Technical Issues: 2.1. In the event of technical issues or errors related to online ticket purchases or membership fee payments, patrons are encouraged to contact Mother Sauce Productions at 860-912-7323 for assistance. 2.2. If an error occurs during the transaction process, Mother Sauce Productions will make reasonable efforts to resolve the issue; however, no refunds will be issued unless there is clear evidence of a double payment or other verifiable errors. 3. Exceptions: 3.1. In exceptional circumstances, Mother Sauce Productions may consider refund requests at its sole discretion. Such requests must be submitted in writing to [refund request email address] and will be reviewed on a case-by-case basis. 3.2. Any refunds granted will be subject to administrative fees and transaction costs incurred by Mother Sauce Productions. 4. Amendments to Refund Policy: 4.1. Mother Sauce Productions reserves the right to amend this Refund Policy at any time. Any changes will be effective upon posting the revised policy on the Entertainment Venue's website. 4.2. Patrons will be bound by the most current version of the Refund Policy for all donations made after the effective date of any changes.
